Hennepin County Asks For Health Care Input

Hennepin County will host a meeting on health care in Minnetonka.

Agencies and service providers interested in learning about Hennepin County’s ideas for a health care demonstration that would test alternative and innovative health care delivery systems are invited to a meeting on Tuesday, Aug. 23 from 9 – 10:30 a.m. at Hennepin County Library- Ridgedale.

The meeting will take place in the library's conference room, lcoated at 12601 Ridgedale Drive.

Hennepin is responding to a request for proposals from the Minnesota Department of Human Services. The goal is to deliver more effective health care. The county provides health and human services to more than 200,000 residents via its cooperative network, which includes Hennepin County Medical Center, NorthPoint Health and Wellness Center, Metropolitan Health Plan and the Human Services and Public Health Department.

The county’s goal is to build upon this tradition of collaboration to create an integrated health care delivery network that blends medical, behavioral health and social services in a patient-centered care model.

Hennepin County is seeking partnerships to fully develop a community model that meets individual service needs, diversity and geographic locations. Initially, Hennepin is focusing on services for adults. Partnerships are sought for the following areas:

-Primary care services

-Chemical health services

- Housing supports

- Vocational services

- Mental health services

- Home health services

Registration for the community forum is required. Organizers ask that people RSVP by 4:30 p.m. Monday, Aug. 15.
